Introduction: Periodontitis is a chronic inflammatory disease associated with dysbiotic biofilm that affects the protective and supporting tissues of the teeth. Scaling and root planing (SRP) is the gold standard treatment, in addition to the adjuvant use of drugs and/or surgical treatment. The use of bisphosphonates in dentistry has been increasingly used to control diseases involving bone resorption. The gels can be easily prepared and administered into periodontal pockets as an adjuvant to SRP and have an impact on improving periodontal parameters. Objective: To analyze the efficacy of 1% alendronate gel in association with scaling and root planning in patients diagnosed with periodontitis. Materials and methods: Articles were selected from the Medical Literature Search and Analysis System Online (MEDLINE), Scientific Electronic Library Online (SciELO), and Google Scholar databases, as well as databases of higher education institutions in Brazil. The following descriptors were used: ‘’Alendronate, ‘’Gel’’, ‘’Periodontitis’’, “Bisphosphonates”, and data collection was carried out in 2023-2024. Results: Eight articles were selected, in which positive results were observed in the therapeutic groups that used 1% alendronate gel as an adjunct to RAR, in which there was a decrease in probing depth, gain in attachment and filling of bone defects. Conclusions: The data analyzed suggests the use of 1% alendronate gel as a promising adjunct to RAR in patients with periodontitis. Further studies are needed to analyze the long-term effects of the drug, its side effects and residual concentration.
